Application Options

Please select your desired enrollment pathway:

1. Fast Track Program (for Reinstatement or Quick Recertification)

Former diplomates whose certification has lapsed (not as a result of a disciplinary action) and non-time-limited certificate holders interested in an expedient pathway to voluntary recertification may apply to the Fast Track Program. Fast Track allows diplomates to obtain recertification or reinstatement within a three‐year window by completing the following steps:

  1. Verify your valid and unrestricted license to practice medicine.
  2. Attest to the completion of at least 250 AMA PRA Category 1 Credits™, including 80 credits that qualify as self-assessment.
  3. Attest to completion of one activity that qualifies as patient safety.
  4. Meet the passing standard on all five available years of Quarterly Questions®.
  5. Complete two activities from the Improvement in Medical Practice Menu.
  6. Submit the $2,000 recertification fee.

If your certificate has lapsed within the past three years (not as a result of a disciplinary action), any activities you completed during your most recent 10‐year cycle may be used to meet Fast Track requirements. However, once the three‐year window has closed, all activities completed during the prior 13 years will expire. Individuals who have been decertified for four or more years may submit an application to begin Fast Track with a clean slate.

2. Annual Program for Voluntary Participation

If you are a non-time-limited certificate holder (certified prior to 1992) who is interested in sampling the Continuing Certification program on a yearly basis, you have the option to enroll in our Annual Program. Successful completion of the program at the end of 10 years will reward you with a voluntary recertification certificate, in addition to the certificate you already have. This program requires you to:

  1. Submit a yearly payment of $200.
  2. Verify that you hold valid and unrestricted licensure.
  3. Attest to the completion of at least 50 AMA PRA Category 1 Credits™ every two years, including 16 credits that qualify as self-assessment.
  4. Attest to completion of one activity that qualifies as patient safety.
  5. Meet the annual passing standard for Quarterly Questions® at least 5 times within a 10-year period.
  6. Complete two activities from the Improvement in Medical Practice menu.
